Joint pain may be from various origins. It may be associated with inflammation, to be called as arthritis. Arthritis may affect both small joints and large joints. Some diseases preferably attack small joints first in preference to the larger joints. In the long run, as disease progresses larger joints are also affected. Arthritis may be a part of a systemic disease or due to a local cause. Some diseases can be suspected right from the involvement of typical joints. There is also typical description of symptoms pertaining to a particular form of arthritis.
Gouty arthritis affects male more than female. It may be due to fault in protein metabolism or excessive production of uric acid due to excessive break down of proteins. It typically affects the joint of great toe to the foot bone; called metatarsophalangeal (MTP) joint. As disease progresses it may affect the joints up, in a progressive manner from heel, ankle, knee, wrist, elbow to shoulder etc. It may not affect same joints of both sides at the same time. The skin of joint appears glossy and may have swellings in it from the deposits of urate crystals called gouty tophi. The joint itself may be swollen. There is restriction of movement. The pain is very much typical in this disease; described variously as excruciating, tearing, bursting and gnawing. It is in its full blowing after the midnight, around 2 AM. In absence of treatment patient is bound to remain awake and irritated. The blood level of uric acid more than 8mg% and 6mg% in male and female respectively is diagnostic of gout with a raised erythrocyte sedimentation rate (ESR). Additionally x-rays examination of the joint, aspiration from the joint fluid for urate crystals may help in establishing the diagnosis. Uric acid crystals may form stone in the urinary system. Those who take excessive meat may suffer from it.
Rheumatoid disease is a systemic disease of autoimmune origin whose hall mark is involvement of joints in addition to involvement of many other systems of body. Arthritis here may be associated with fever, swelling of involved joint and crippling pain. The disease may affect eye, lungs, skin, spleen heart and kidneys etc. It typically affects symmetrical joints i.e. same joint in both the sides; to start with the proximal interphalangeal joint, the first joint of a finger. It prefers to affect the ring finger first. The joint becomes spindle shaped, later on deformed and stiff. The finger may appear as swan neck, to be described as swan neck deformity. As disease progresses; it may affect the larger joints like knee, elbow, hip and shoulder etc. A highly raised ESR with positive rheumatoid factor(RF) in blood and associated arthritis are diagnostic of the disease.
Another arthritis called osteoarthritis, though not arthritis per Se, as there is no inflammation; can be better termed as osteoarthrosis. Here the pain is mostly due wear and tear in the soft parts of joint, the cartilage; from an aging process. The joint typically affected are distal interphangeal joints of fingers, the last joint of a finger. It may be swollen, to be called as haberdens nodes and may not be symmetrical. As disease progresses other joints like knees and hips may get involved. Diagnosis is based on exclusion of other forms of arthritis, old age of the patient and changes seen in x-rays examination of joints.
The first two forms have some specific treatment where as the last named poses problem in satisfactory treatment. Simple analgesics, physiotherapy and some dietary supplements may keep it away from crippling the patient. In the long run, it may require a joint replacement.
what will doctors ask me?
after being refused disability living allowances, i have asked to see a nhs dr , i have severe osteoarthritis’s in my hands hip and rib gage .i haveplanovalgus deformities of both ankles and a hallux bilaterally .i have severer psoriasis on the soles of my feet and need both bunions removed shortly ,i have asthma and get out off breath very easily .i am 64 yrs old and feel very let down as i worked in catering and had to retie re at 59yrs because of these illness.i don’t smoke drink and not over weight at 9st 6 . will a dr be sympathetic towards me i don’t believe in hand out as i worked all my life but i am at home alone most of the time and do need help.
i dont think you understand my question i am not over wieght and that is not my problem.
The beneifit system is there to help the people that need help, i think that once you have seen a dr and he gives you the note to give to the job center that you will get your allowances, they may even be back dated for the weeks you have missed.
